looking for thin flat pen.... I have seen this pen several times... can't remember what supplier has it...

There are a few options:

Jornik Manufacturing has a flat pen that is also a bookmark or paperclip.
Magnet LLC has a magnetic flat pen.
Riverplace Marketing has a flat pen with staple remover and magnetic back.
Stencil Co has a flat refillable ballpoint pen.
Supermatic has a pen refill within a vinyl sleeve, but I did not see just the pen - it is coupled with a notepad.

Forgot to mention - received a sample of the Prime Resources magnetic flat pen. The house shape top is PL1111 - there is also a star PL1112 and a one PL1113. The sample writes very well.

The guy at Prime told us these are in high demand this year. That just seems funny considering how old these things are. ;)

This probably doesn't matter now, but I just noticed that I have one stuck on the file cabinet from Points of Light. I looked them up and they have a bunch of industry specific heads:
